Forms can be found outside of the Religious Education Office. Forms must be signed and notarized and dropped off to the Religious Education Office with a check for $125, made out to Blessed Sacrament Church. Teams consist of a minimum of nine players. If you are looking to start a new team, a coach will be needed. Coaches must be fingerprinted and VIRTUS trained. Any questions, please contact Gretchen English at gruenglish@verizon.net.
